RESUMO

Left ventricular free wall rupture is a fatal complication of myocardial infarction for which infarctectomy and reconstruction of the left ventricle using a prosthetic patch under cardiopulmonary bypass are performed. However, these surgical treatments remain challenging. Left ventricular free wall rupture secondary to acute myocardial infarction was diagnosed in an 86-year-old man. We performed sutureless repair of the left ventricular free wall rupture without cardiopulmonary bypass. During the operation, a pre-gluing bovine pericardial patch with Hydrofit® was placed twice on the ruptured site and manually pressed to provide complete hemostasis. The postoperative course was uneventful. This sutureless technique has the benefit of avoiding sutures in the fragile infarcted myocardium and might be effective for left ventricular free wall rupture treatment.

In the surgical treatment of acute type A aortic dissection, performing the anastomosis in the dissected aorta is technically challenging due to the fragility of the dissected aortic wall. This study describes the reinforcement technique for the distal anastomotic site, using pre-glued felt strips with Hydrofit®. No intraoperative bleeding occurred at the anastomosis site of the distal anastomosis stump. Postoperative computed tomography revealed no new distal anastomotic entry. This technique is recommended for managing acute type A aortic dissection during distal aortic reinforcement.

RESUMO

When performing translocated total arch replacement using a frozen elephant trunk, the closure of supra-aortic vessels' stump may be necessary. Herein, we describe the application of a nonabsorbable polymer Hem-o-lok clip for the closure of the stump. The supra-aortic vessels were looped using cloth tapes and clipped using Hem-o-lok clips with gentle traction of cloth tapes. Then, the supra-aortic vessels were sharply transected, leaving a 2-mm cuff of tissue. There were no incidents of clip failure during application or migration, and no significant intra-operative or post-operative bleeding occurred due to clipping failure. Hem-o-lok clips are an easy, quick, and reliable method for the closure of supra-aortic vessels' stump. Therefore, we recommend this technique for translocated total arch replacement procedures performed using a frozen elephant trunk.

RESUMO

An 80-year-old woman was hospitalized for aortic valve insufficiency, paroxysmal atrial fibrillation, and ascending aortic aneurysm. She underwent aortic valve replacement, pulmonary vein isolation, left atrium appendectomy, and ascending aorta replacement. She developed a subcapsular hepatic hematoma during the surgery. The patient was managed conservatively and discharged successfully.

RESUMO

A 70-year-old man had an acute type B aortic dissection 9 years before his admission. The last enhanced computed tomography that was performed revealed an aneurysm that extended from the ascending aorta to the aortic arch, associated with a chronic aortic dissection, which extended from the aortic arch to the left external iliac artery. His visceral arteries originated from the false lumen. We performed a total arch replacement with a frozen elephant trunk in the hybrid operating room. Immediately after the circulatory arrest termination, using intraoperative angiography, we verified that the blood supply to the visceral arteries was patent.

OBJECTIVE: The present study aimed to evaluate the outcomes of total aortic arch replacement with proximalization of distal anastomosis using the frozen elephant trunk technique with the J Graft FROZENIX (Japan Lifeline, Tokyo, Japan) and Gelweave Lupiae (Vascutek Terumo Inc, Scotland, United Kingdom) graft (distal anastomosis performed in zones 1 and 2) in patients with acute Stanford type A acute aortic dissection. METHODS: A total of 50 patients underwent total aortic arch replacement using the frozen elephant trunk technique, deploying the J Graft FROZENIX into zone 1 or 2 (zone 1: n = 17, zone 2: n = 33) in combination with the Gelweave Lupiae graft for acute Stanford type A acute aortic dissection. Patient characteristics, intraoperative data, and early and midterm outcomes were analyzed. RESULTS: The overall in-hospital mortality rate was 4% (2 patients). The in-hospital mortality rate in patients with visceral malperfusion was 11% (1/9). There were no patients with paraplegia and stent graft-induced new entry. Resection or closure of the most proximal entry tear was achieved in 100% of 42 patients who had postoperative computed tomography. The overall survival was 87.9%, 84.1%, and 84.1% at 1, 2, and 3 years, respectively. However, 1 patient required endovascular extension for the dilatation of the descending thoracic aorta 4 months after the initial surgery. CONCLUSIONS: Total aortic arch replacement with the frozen elephant trunk technique (zone 1-2) and Gelweave Lupiae graft was safe and effective in simplifying surgery for acute Stanford type A acute aortic dissection.

Ruptured acute type B aortic dissection is a life-threatening condition with a high mortality rate. Right hemothorax secondary to this condition is extremely rare. Herein, we report a successful treatment of a ruptured acute type B aortic dissection via thoracic endovascular aortic repair in a 45-year-old man who initially presented with right hemothorax. Contrast-enhanced computed tomography confirmed massive right hemothorax and acute type B aortic dissection in which the primary entry was located just below the left subclavian artery. Moreover, a possible rupture site in the descending aorta at the level of Th6 was identified. We then performed an endovascular aortic repair with left subclavian artery open surgical debranching. His postoperative course was uneventful. The patient did not have any complications at a 6-month follow-up.

RESUMO

Aortoenteric fistula after endovascular aortic repair for an abdominal aortic aneurysm is a rare but severe complication. Particularly, a case of inflammatory abdominal aortic aneurysm is extremely rare and there are only 3 reported cases. A 70-year-old man underwent endovascular aortic repair for impending rupture of an inflammatory abdominal aortic aneurysm and was medicated steroids for approximately 2 years. Four years after endovascular aortic repair, he developed endograft infection with an aortoduodenal fistula and a left psoas abscess. He underwent total endograft excision, debridement, in situ reconstruction of the aorta using prosthetic grafts with omental coverage, and digestive tract reconstruction to prevent leakage. Pseudomonas aeruginosa was detected in the infected aortic sac. The patient has not experienced recurrence of infection in the 35 months since his operation.

RESUMO

A right ventricular cardiac tumour was incidentally detected in a 50-year-old woman during medical check-up. Transthoracic echocardiography, computed tomography and magnetic resonance imaging revealed that the right ventricular tumour was widely attached to the free wall of the right ventricle and extended to the right ventricular outflow tract. The tumour was excised surgically, and the postoperative course was uneventful. Histological examination revealed that the tumour was a cardiac haemangioma.

While there are many reports on partial aortic root remodeling, it is rarely performed for chronic aortic dissection of the coronary artery. This report presents a case of a 69-year-old man incidentally diagnosed with aortic dissection during routine checkup. He had a history of percutaneous coronary intervention from the left main trunk to the left anterior descending artery and left circumflex artery. Computed tomography revealed a chronic type A aortic dissection with an aneurysmal aortic root. The false lumen of the Valsalva sinus originated from the left anterior descending artery and expanded largely to the non-coronary Valsalva sinus. We performed partial aortic root remodeling, resecting the dissected non-coronary Valsalva sinus. The postoperative course was uneventful. Partial aortic root remodeling was effective, but its use might be controversial for chronic aortic dissection without resection of the primary entry of the left anterior descending artery. Moreover, close follow-up is required.

RESUMO

An 80-year-old woman underwent aortic valve replacement and ascending aortic replacement. Two years later, computed tomography revealed a pseudoaneurysm of the ascending aorta replaced with a prosthesis. The pseudoaneurysm arose from the stump of a side branch of the prosthesis. Endovascular treatment for the pseudoaneurysm was carried out using the aortic extension cuff of an infrarenal endovascular system. The postoperative course was uneventful.

RESUMO

A 67-year-old woman encountered a traffic accident and had chest computed tomography(CT) examination. It revealed a 24 mm Kommerell's diverticulum associated with a right-sided aortic arch and a 15 mm saccular aneurysm of an aberrant left subclavian artery. We performed intra-aneurysmal coil embolization for the left subclavian artery aneurysm after a balloon occlusion test of the left subclavian artery. The postoperative course was uneventful.

RESUMO

We describe the case of a 46-year-old man with pulmonary thromboembolism caused by a popliteal venous aneurysm with thrombosis. The aneurysm was fusiform and partially saccular with a thrombus, and the caliber of the native popliteal vein was large. Tangential aneurysmectomy with lateral venorrhaphy was difficult because of the aneurysm type, and graft interposition was required because of the large venous diameter of the anastomosis site. The patient underwent aneurysm resection interposed with a spiral saphenous vein graft. The postoperative course was uneventful, and the graft was patent at 1 year after surgery.

RESUMO

A 38-year-old man with polyarteritis nodosa presented with exertional chest pain. Coronary angiography revealed 3-vessel coronary artery disease. We performed 2-vessel coronary artery bypass grafting with the saphenous vein because the left internal thoracic artery was in poor condition. Histological examination of the left internal thoracic artery showed a strong effect of polyarteritis nodosa.

A 40-year-old woman who was diagnosed to have systemic lupus erythematosus developed complications of cerebral infarction and alveolar hemorrhage. Close examination revealed severe aortic insufficiency, and she was diagnosed with Libman-Sacks syndrome. Due to progressive dilatation of the left ventricle and her easily fatigued state, surgery was performed. On pathological examination, holes of 4-mm and 5-mm diameter were detected in the left coronary and noncoronary cusps of the aortic valve, respectively. The morphology of the valve lesions showed a characteristic shape of such huge holes.
